This is to reiterate that effective May 10, 2010 our new insurance provider is Prudential Guarantee and Assurance, Inc. through BDO Insurance Brokers, Inc. with the following benefits;
Accidental Death & Disablement - P 200,000.00
Permanent Total Disability - P 200,000.00
Unprovoked Murder & Assault - P 50,000.00
Accidental Medical Expense - P 10,000.00
Accident Burial Benefit - P 10,000.00

The eligible age for insurance coverage is 18 to 65 years old only. You are covered 24 hours a day, 365 days a year. However, MOTORCYCLING is excluded under this program.
